PREPARING FOR YOUR APPOINTMENT
New Patients & Insurance Participation
Before your appointment, please:
- Confirm that Arthritis & Osteoporosis Treatment Center is a participating provider with your insurance plan
- Bring your insurance card and a valid photo ID
- Be prepared to pay any applicable copay or patient responsibility
If you are a new patient, completing required electronic forms in advance and arriving prepared will help reduce wait times and ensure a smooth first visit.
NEW PATIENTS
To reduce wait times for your initial visit:
- Please complete all required electronic forms prior to arrival, whenever possible
- If forms are completed in advance, arrive 15 minutes early
- If you need assistance completing forms, please arrive 30 minutes early
What to Bring to Your Appointment
Please bring:
- Your insurance card (required at every visit)
- A current list of medications or the medications themselves
- Copies of recent office visit notes, lab results, and diagnostic tests
- Imaging studies from the past year (X-rays, MRIs, bone density studies), including reports and discs when available

RETURNING PATIENTS
To help make your visit as efficient as possible:
- Present your insurance card at each visit and be prepared to pay any copay or outstanding balance
- Notify the front desk of any changes to your address, phone number, or insurance
- Inform staff and your provider of any changes in medical or surgical history
- Bring an updated medication list or your medications
- Request prescription refills during your visit
Electronic Check-In & Digital Forms (No App Required)
To streamline your visit, our practice offers multiple digital check-in options that do not require downloading an app.
You may complete all required forms using:
- The check-in link sent by text message prior to your appointment
- An in-office QR code
- Our check-in via iPads in the office
These options allow you to complete paperwork and check in quickly, even if you do not use the healow app.
Patient Portal & healow® App (Optional but recommended)
In addition to appointment check-in, our practice offers a secure Patient Portal powered by healow®, allowing you to manage aspects of your care between visits. The Patient Portal and healow app enable patients to communicate with our practice easily, safely, and securely over the internet.
What You Can Do with the Patient Portal / healow App
Once activated, you may:
- View your medical history, including:
- Problem list
- Allergies
- Immunization records
- Medication list
- Lab, imaging, and procedure results (after provider review)
- Request prescription refills for authorized medications
- Schedule, reschedule, or cancel appointments
- Receive appointment confirmations and reminders
- Access health records for authorized family members
Accessing the Patient Portal
Option 1: Web Browser (No App Required)
You may log in directly through your web browser using the secure portal link provided by our practice.

Login credentials are sent to the email address on file with our office.
Option 2: healow® Mobile App (Optional)
The healow app provides the same portal features in a convenient mobile format.

- Download the free healow app from the Apple App Store or Google Play Store
- Search for our practice using our unique Practice Code Practice Code: CJHJDD
- Log in using the portal credentials provided by our office
If this is your first time using the Patient Portal or healow app, or if you are a new patient, our staff must activate your account. Please call our office at (904) 276-0001. Our team will be happy to assist you with activation or password resets if needed.
After the visit if you require any results to be faxed to your primary care or other physicians, please provide us with the name, phone and fax number of the physician.
